Medicare Eligible Providers (EP)

EPs for the Medicare program can qualify for up to $44,000 per provider over a 5-year period beginning January 1, 2011. Medicare fee for service (FFS) EPs include:

• Physicians (MDs and DOs),
• Dentists (DDS and DDM),
• Podiatrists (DPM),
• Optometrists (OD)
• Chiropractors (DC)

Medicare Advantage (MA) eligible professionals include those professionals listed above that can document they provide, on average, at least 20 hours/week of patient care services.¹
