Eligible Provider (EP) Registration

How does one register to become an EP?

EPs (and eligible hospitals) are required to register with the Centers for Medicare & Medicaid Services (CMS) via the EHR Incentive Program website. EPs may be enrolled in Medicare Fee for Service (FFS), Medicare Advantage (MA), or Medicaid (FFS or managed care). Each EP provides their name, National Provider Identifier (NPI), business address, phone and tax payer ID number (TIN). The EHR Incentive Program website became operational to accept information on January 3, 2011.

Please note that although the Medicaid EHR Incentive Programs began on January 3, 2011, not all states were ready to participate on that date. Information on start dates and when registration will be available for state Medicaid EHR Incentive Programs in each state is posted at: http://www.cms.gov/EHRIncentivePrograms/40_MedicaidStateInfo.asp.
